She failed to follow instructions for whatever reason. If a teacher gives you scizzors and asks you to cut paper, is the teacher to blame when you cut off a finger? If an instructor tells you to do something and you don't understand the instruction, one should ask questions until they do. This is where things go horribly wrong, and why people feel it's important to post "no diving" on the side of two ring 6 inch deep baby pools. OBVIOUSLY you don't dive into a pool like that, but somebody did, and the pool company got sued because people couldn't use common sense.
